Paypal mostly. Snail mail payment considered if asked about it first... but isn't that banned now?! Too bad for people who don't have a card but good for anyone who may get scammed. Geez, that's horrible some bad eggs had to ruin it for the rest of us.

I try to get things out within 2 business days of payment but I'm in school and also I live in a rural part of Marin called Point Reyes and our post office isn't open Saturday, so sometimes it takes me 3-4 days. I only use USPS service. Sometimes I throw in free tracking or insurance but if I don't specify on the listing, it likely means it's optional.

Not generally. Exception may be given if I really make someone mad. I'd much rather prefer to be contacted if one is unsatisfied than get positive feedback with negative comments. I try my best to be honest about all flaws in the ads and start with a low price if that's the case. Sometimes I do low starting bids on perfect condition items however because I want ebayers to pay what they think something is worth.
